Stewardship Forms Online!


    STEP 1: 
              Read message from Fr. Mark & our school principals

 STEP 2:  Complete the Form(s):
               Families, complete the:
               Family Stewardship Form 

               Family Stewardship Form (Mac Version)
          
               EACH student in grades 6-12 next school year (2012-13)
               who is not currently attending TFS*, complete the: 
               Student Stewardship Form
                  *TFS Students will complete their forms in class    
  
               (See bottom of page for tips and help with online forms)
      
                 
   STEP 3:    Print, Sign and Submit
               When the Stewardship Forms are completed:

- PRINT the forms 
  (Print button is at the top and bottom of each form)
- SIGN & DATE where indicated
- PLACE in an envelope marked "Confidential"
- RETURN to the parish office, addressed to:
             Fr. Mark c/o Joan Port
             St. Francis of Assisi 
             11401 Leesville Rd
             Raleigh, NC 27613
   Please do not return forms to
   the school office.

 Tips about the Stewardship Forms: 

                   1) You will need Adobe Acrobat Reader to open & fill out
                        these Forms.  MAC users might have to save the .pdf file
                        to the desktop or download folder and open with Adobe Reader.

       

2) Form fields must be filled out ON LINE for legibility.
    Please DO NOT print it and fill in by hand.

3)  We recommend you click on the 'Highlight Fields' button in the
      top right-hand corner in Adobe Reader.
       
     This will outline all the fields to be filled in & will highlight the
     required fields in red.
